ENROLLMENT PROCEDURE FOR UNDERGRADUATE
I. COLLEGE FRESHMEN
STEP 1. Fill up the Application Form HERE: bit.ly/AHCSEnrollmentForm
List of Admission Requirements
* Original Copy of High School Report Card (Form 138 / 137) or JHS Report Card
* Original Copy of Certificate of Good Moral Character
* Photocopy of Birth Certificate ( PSA )
* Church Recommendation Letter
*. 2pcs. ID Pictures (2 x 2)
* Personal Essay or Life Testimony
STEP 2. Take the AHCS Admission / Entrance Test
STEP 3. Entrance interview with the Admission Committee / Dean / Registrar
STEP 4. Fill-up waiver form (if lack requirements)
STEP 5. Pay the downpayment - Cashier
STEP 6. Student Orientation

* Honorable Dismissal
* Certificate of Good Moral
